Forged Logs, Missing Signatures, Phantom Cellmate

The BOP investigation found 179 of 183 required guard signatures missing from the suicide watch log. A Drug Treatment Specialist forged entries for rounds that were not completed. Guards admitted to investigators that they had not completed their required checks while paperwork indicated otherwise. Epstein was placed in a single cell despite BOP policy requiring a cellmate after his first alleged suicide attempt six weeks earlier.

The FBI Takes the NVR

Two days after death, FBI took MCC's recording equipment. Staff asked: "So they're taking the NVR?" Recording capabilities were offline 8-10 days. 15-20 of 161 cameras were already non-operational. FBI agent texts: upper management "didn't want anything related to Epstein put into writing."

Records indicate Epstein's last phone call was placed to his mother — who had been dead since 2004. FBI agent text messages, later disclosed in litigation, reveal that upper management at the Bureau directed staff to communicate about the Epstein case only "via word of mouth" — no written records, no emails. The combination of missing footage, forged logs, and deliberate oral-only communications has never been resolved by any official investigation.
