Good afternoon. Dr. Shimony is requesting assistance in scheduling Mr. Epstein for his nuclear test of his parathyroid. He is willing to selfpay for this test and asks if he can be accommodated this Wednesday 12/13/17 any time will do. He will be flying in from Europe

We can accommodate the day, but prior visit information shows he has active United Health Care. A pre-certification or a written denial is required. Without the above, if the patient submits an invoice to a carrier we participate with, the insurance would deny payment and require us to give the patient a full refund for not following our contract. Please note it's a 3 hour procedure.

Can patient sign a form which waives his insurance rights?

Unfortunately, not. This has happened on a prior occasion - a very well to do patient put us in the same unfortunate situation. He paid on day of service because he did not want to wait, and not only came back and asked for his money back, but then came back to the practice for his follow ups.
