FW: tax form

I was not sure to whom I should write about this, but I am writing to inquire about the payment I received from Jeffrey last spring after the conference arranged on St John. I received a gift that will not be taxed, but also a bonus for which I will need a 1099M. I have not yet received this form (whether because it went to the wrong address or has not been sent, yet), but I would be most grateful if it could be sent to the following address at your earliest convenience:
