As I discussed with you we do need to have visa information for the Norwegian passengers going to Bermuda on May 6th. Also we need to verify that both biological parents are traveling with the minors. If not we will need the attached minor consent form completed and notarized by absent parent prior to departure. Please fax a copy to us at before the trip and the original needs to be carried on board the flight. In addition please provide the following information:
