HI Jennifer and thank you for getting me the paper work. I plan to hand this to Jeffrey before he bolts out the door, but just in case, I am sending this to you as well! I've included a copy of his Insurance Card. He still needs to sign and fill out the personal questions...I have done as much as I can for him now!

Jennifer! Don't kill me!! Jeffrey now says he is going to Palm Beach so we will want the prescription called in to a pharmacy there...working on getting you details for one....I am not sure who we have used in the past...but I'm sure there are lots of drug stores around...sorry!
