HI Jennifer and thank you for getting me the paper work. I plan to hand this to Jeffrey before he bolts out the door, but just in case, I am sending this to you as well! I've included a copy of his Insurance Card. He still needs to sign and fill out the personal questions...I have done as much as I can for him now!
Mr. Epstein would like to come in next Thursday for a treatment with Laura. What is his availability on the 17th? Also see attached for his statement for today.

Yes, he will come in at 10am and numb for his 10:30 with Laura. Please keep in mind that her schedule has very limited flexibility due to the length of her appointments. If by chance he is running late please let me know ASAP.

Jeffrey now says he is going to Palm Beach so we will want the prescription called in to a pharmacy there...working on getting you details for one....I am not sure who we have used in the past...but I'm sure there are lots of drug stores around...KwiLet me know once Dr. Kline approves the prescription. thanks,
