Hi Nicole...can you please fill out this form Dr. Woody Merrell's office needs in order to try and get you in today to see him..the faster we can get this form filled out the better...you can send directly to Candice in his office ...please let me know once done so I can follow up with her re an appointment today...i have explained this is an emergency as Jeffrey requested...however, I do know the doctor has some other emergency today already and is leaving the office early..they are cancelling appointments already..but hopefully we can get you in before he leaves ...thanks!
